Apartment Supports Counselor

QUALIFICATIONS:
B.A in Behavioral Sciences plus experience preferred. Alternative education and experience may be substituted for degree. High School Diploma and valid drivers license required.

SUPERVISION:
Position is supervised by the Program Coordinator.

JOB DUTIES:
1. Assist in developing and implementing individualized service plans.
2. Assist individuals to link to, manage and coordinate all community resources.
3. Facilitate the development of valued community roles and relationships.
4. Assist individuals with completing daily routines.
5. Ensure health and safety of individuals and the cleanliness of their apartments.
6. Implement all prescribed medical and medication protocols.
7. Assist individuals to access and/or provide all needed transportation.
8. Participate in team meetings, supervisions, and other assigned meetings.
9. Review and complete all required documentation.
10. Communicate pertinent information to stakeholders.
11. Assist individuals to oversee their PCA Program, if applicable.
12. Provide crisis intervention as needed and participate in regularly scheduled on-call beeper rotation.
13. Acquire and maintain all required certifications.
14. Perform other related tasks as assigned.

Counselor, DDS Residential Services (40, 30 & 20 hrs AM)

Develop and implement Individual Support Plans/meaningful goals for people served. Collect data and report on outcomes, provide support with doctor appointments, medication administration, meal preparation, and other daily routines. Create opportunities for meaningful community involvement. Provide transportation and assistance with personal care/behavior management as needed. Implement all procedures that ensure physical safety and cleanliness of the program.
