Program Coordinator

Develop and implement component goals, objectives, and work plan tasks that are directed toward accomplishing the Agency’s Mission and Vision, provide goal oriented supervision to Counselors and Program Supervisor, facilitate weekly staff meeting, develop and manage program budget. Ensure optimum program coverage through the development and monitoring of the program schedule. Ensure the development and implementation of all habilitation, whole life, and other service plans, facilitate and monitor the implementation of activities that promote the development of valued roles and personal relationships in the community, develop and monitor the implementation of all procedures that ensure the health, safety, and well being of individuals served, monitor the implementation of all prescribed medical and medication protocols. Participate in the regular rotation of an emergency pager.

Apartment Supports Counselor

QUALIFICATIONS:
B.A in Behavioral Sciences plus experience preferred. Alternative education and experience may be substituted for degree. High School Diploma and valid driver's license required.

SUPERVISION:
Position is supervised by the Program Coordinator.

JOB DUTIES:
1. Assist in developing and implementing individualized service plans.
2. Assist individuals to link to, manage and coordinate all community resources.
3. Facilitate the development of valued community roles and relationships.
4. Assist individuals with completing daily routines.
5. Ensure health and safety of individuals and the cleanliness of their apartments.
6. Implement all prescribed medical and medication protocols.
7. Assist individuals to access and/or provide all needed transportation.
8. Participate in team meetings, supervisions, and other assigned meetings.
9. Review and complete all required documentation.
10. Communicate pertinent information to stakeholders.
11. Assist individuals to oversee their PCA Program, if applicable.
12. Provide crisis intervention as needed and participate in regularly scheduled on-call beeper rotation.
13. Acquire and maintain all required certifications
14. Perform other related tasks as assigned.
